What We Do / How We Help with Eddy SeniorCare:

  • brings a full range of supplies, equipment and help to your home
  • provides in-home personal care, nursing, therapy and social work services to help you remain independent
  • provides transportation to medical appointments
  • gives you "peace of mind" knowing there's a dedicated team to coordinate all your care
  • gives you opportunities to make new friends and socialize with other seniors in our day center, where you can also enjoy a healthy, delicious lunch

Eddy SeniorCare is the Capital Region's only Program of All-Inclusive Care for the Elderly (PACE). PACE is a special model of care proven effective at helping older adults with long-term health care needs live safely at home.

Eddy SeniorCare supports seniors at home by bringing together under one roof a wide array of medical care, home health care, therapy, and other support services. In the PACE model of care, each participant's health and support services are managed by a care team that is based at our agency. The team includes a doctor, a registered nurse, therapists, home health aides, day center staff, transportation drivers, nutritionists, social workers, and others. The PACE "one-roof" team model of care improves care coordination and responsiveness.

Behavioral Health Services

To request assistance scheduling behavioral health services and/or transportation to and from appointments, please contact:

  • Off hours, weekends and holidays: On call: 518-382-3290 and ask for on call Registered Nurse

What We Offer

• Comprehensive Behavioral Health Services for PACE participants
• Access to In-Network Providers for psychiatry, psychology, therapy, and substance use disorder (SUD)
• Telehealth Options available for convenience and continuity of care
• Cultural & Linguistic Support including interpreter services at no cost

Access Standards

• Initial Appointment: Within 10 business days
• Post-Hospital Discharge: Within 7 calendar days
• Urgent Needs: Immediate escalation for emergencies or urgent behavioral health concerns
• If these timeframes cannot be met with in-network providers, PACE will authorize out-of-network services, at no additional cost to the PACE participant.

• Transportation Coordination (including escorts if needed)
• Language Assistance for non-English speakers and accommodations for hearing/vision impairments

Filing a Complaint or Grievance

Participants or caregivers can file a grievance orally or in writing. Language assistance and accessibility accommodations are available.
