Bariatric Surgery vs. GLP1 Medications
Bariatric Surgery vs. GLP1 Medications: Which Weight Loss Treatment Is Right for You?
A Trusted, Comprehensive Approach to Long-Term Weight Loss
Struggling with your weight can feel overwhelming, especially with the surge of GLP1 weight loss medications like Ozempic and Wegovy in the spotlight.
But how do these medications compare to bariatric surgery, and which option delivers the most lasting results?
At. St. Peter’s Health Partners, we provide a holistic, medically guided weight loss program offering both surgical and medication-based solutions - personalized to your unique health needs and long-term goals.
What Are GLP1 Medications?
GLP1 medications (glucagon-like peptide1 agonists) were originally developed for diabetes but are now widely used for weight management.
Available as tablets or injections, these medications:
- Slow digestion to help you feel full longer
- Reduce cravings and appetite
- Improve insulin response
- Support modest yet meaningful weight loss
Benefits of GLP1s
- Noticeable weight loss
- Improved blood sugar control
- Reduced risk factors for heart disease
- Lower cholesterol and improved blood pressure
- At-home, nonsurgical treatment
Limitations
- Weight often returns after stopping
- Less total weight loss than surgery
- Common GI side effects (nausea, vomiting, diarrhea)
- High out-of-pocket costs
- Muscle loss potential
- Rare but serious risks: pancreatitis, gallbladder disease, kidney injury
Who Is a Good Candidate for GLP1 Medications?
GLP1s may be right for adults with:
- BMI ≥ 30, or
- BMI ≥ 27 with a related condition (Type 2 diabetes, high blood pressure, high cholesterol, cardiovascular disease, or sleep apnea)
- Patients must be willing to commit to long-term lifestyle changes and understand that many people (60 to 80%) cannot tolerate GLP1s long-term.
What Is Bariatric Surgery?
Bariatric surgery works by reducing stomach size or rerouting digestion to limit food intake and decrease appetite-regulating hormones. We offer two minimally invasive surgical options:
Laparoscopic Sleeve Gastrectomy
Removes about 70% of the stomach, creating a smaller stomach pouch that helps patients feel full quickly.
Laparoscopic Gastric Bypass
Creates an even smaller stomach pouch and reroutes a portion of the small intestine to support more significant weight loss and metabolic improvement.
Benefits of Bariatric Surgery
- Rapid and significant weight loss
- Longterm success proven across decades
- Improvement or resolution of Type 2 diabetes, heart disease, reflux, and sleep apnea
- Reduced mortality and improved quality of life
- Insurance coverage available for qualifying patients
Considerations
- Requires short-term pre and post surgery liquid diet
- Recovery includes temporary fatigue and incision discomfort
- Some patients experience sagging skin or hair thinning
- Requires 3 to 6 months of medical preparation and screening
Who Is a Good Candidate for Bariatric Surgery?
You may qualify for weight loss surgery if you have:
- BMI ≥ 40, or
- BMI 35 - 40 with a weight-related condition such as diabetes, hypertension (not controlled on 2-3 blood pressure medication, and sleep apnea (OSA).
Candidates must be healthy enough for surgery and complete all preoperative evaluations.
GLP1 Medications vs. Bariatric Surgery
Which Option Supports the Best Long-Term Weight Loss?
Both GLP1 medications and bariatric surgery can help patients lose weight - but research consistently shows differences in long-term success:
GLP1 Medications
Average ~20% excess weight loss for many patients
Weekly doses are required long-term
May cause gastrointestinal side effects
Weight regain is common after stopping treatment
Often not covered by insurance, costing hundreds per month
Bariatric Surgery
60 to 70% excess weight loss with the highest long-term success rates (according to recent studies)
Onetime intervention with durable results
Shown to significantly reduce or resolve Type 2 diabetes, sleep apnea, hypertension, and more
Often covered by insurance after required evaluations
Can improve or eliminate acid reflux
In some instances, patients may benefit from surgery and GLP1 therapy. It’s important to work with a medical provider who can help.
