Prescription Assistance Program
Helping Patients Gain Access to Prescription Drugs
St. Peter's Health Partners Prescription Assistance Program assists patients in obtaining long-term prescription medications that they may otherwise not be able to afford. This is possible by accessing free or low-cost programs through pharmaceutical companies. The program is available for individuals who are uninsured or underinsured and meet specific income guidelines.
Schedule an enrollment appointment by calling (518) 268-6442 or ask your provider to complete a Prescription Assistance referral form. At your appointment, be sure to bring the following:
- Proof of income
- Copy of most recent federal tax return (if available)
- A list of all current medications
