View OpeningsLocated in Guilderland, NY, Our Lady of Mercy Life Center is a 160-bed nursing center offering long-term skilled nursing care as well as short-term sub-acute rehabilitation services.
Our special rehab unit offers short-term tailored rehabilitation to individuals following a hospital stay so they can recover and get back home. Our patient outcomes for rehab regularly exceed state benchmarks.
Our Lady of Mercy Life Center also offers a special memory care unit for dementia residents who have special needs beyond what regular nursing home residents require. We provide a safe secure area and dedicated care staff who understand the unique needs of those living with dementia.
We are also proud to be one of the only nursing homes in the country to offer two special technological advancements in our memory care unit:
- Our memory care dining room features an artificial sky that mimics the outdoor sky during various times of the day, complete with moving clouds and birds flying by. This technology effectively brings in the light of the outdoors which can positively impact residents, especially those with dementia. Research has shown that exposure to daylight can have a calming health effect, improving mental engagement, reducing potential falls, and aiding in the regulation of the body’s circadian rhythm and sleep and wake cycles.
- Four rooms on our memory care unit feature special “smart” cell cushioned flooring that helps reduce the impact of injuries sustained from falls. This innovative technology is especially important because seniors are at a high risk for injuries from falls.
Our Lady of Mercy Life Center is proud to be part of St. Peter’s Health Partners, a non-profit health system which is licensed as a co-operator of our skilled nursing residence. SPHP’s parent, Trinity Health, is licensed as a co-operator. Both also provide us with administrative services.
Our partners/vendors: As a facility that receives Medicare and Medicaid funding, we want to be transparent regarding which companies we contract with to provide good and services. Currently, at least 90% of the money the St. Peter's Health Partners' nursing homes spend with vendors is spent with five companies: 3M, Cardinal Medical Products, Ecolab Healthcare, Medline Industries, Nova Biomedical and Smith+Nephew. All of these vendors supply personal protective equipment (PPE) for staff and/or medical and personal care products for our residents.
As of March 1, 2026, our rates are as follows:
$486 per day for a private room in long-term care
$472 per day for a semiprivate room in long-term care
$655 per day for a private room in short-term rehab
$640 per day for a semiprivate room in short-term rehab
**Room rates DO NOT include applicable New York State Assessment Tax.
Ownership
Our Lady of Mercy Life Center is proud to be part of St. Peter’s Health Partners, a non-profit health system which is licensed as a co-operator of our skilled nursing residence. SPHP’s parent, Trinity Health, is licensed as a co-co-operator. Both also provide us with administrative services.
Landlord
Our Lady of Mercy Life Center/St. Peter’s Health Partners owns our building and property. We do not rent or lease; we do not have a landlord.
